Upon launching the application, you will be presented with a screen that pre-configures various aspects of the application, such as the initial data load. After a few seconds you will be redirected to the home screen.
The dashboard provides an overview of the number of task assessments and hazards submitted for the current day, week, and year.
The screen presents an overview of all task assessments. There is a search function that allows you to search for specific words within the task or location fields of a task assessment. This feature enables you to easily find safety observations based on specific criteria or keywords of interest.
For each task assessment item, you will notice an orange arrow (>) icon that allows you to view and edit the details of that specific s task assessment. By clicking on the orange arrow, you can access and make modifications to the information associated with the task assessment.
To add a new task assessment, click on the “+” icon.
To proceed with the submission, please follow these steps:
Fill out all the required fields in the form, ensuring that all necessary information is provided accurately.
Once all the fields have been filled out, click on the "Next" button to proceed to the next step of the submission process.
Please note that your GPS location data will be automatically populated based on the functionality of your device. This means that the form will automatically capture your GPS location without requiring manual input.
As you progress through the submission, you will notice a progress bar that indicates where you are in the submission process. This bar helps you track your progress and provides a visual representation of your advancement.
By following these instructions, you can fill out all the fields, click on the "Next" button to proceed, and rely on the auto-population of GPS location data. Additionally, the progress bar will guide you through the different stages of the submission process.
When systematically going through all the task prompts, please follow these instructions:
Click on the appropriate "Yes" or "No" buttons to indicate your answer for each task prompt. Select "Yes" if the task prompt has been fulfilled or completed as required. Select "No" if the task prompt has not been fulfilled or additional action is required.
If you click on the "No" button for a task prompt, a "STOP" alert will be issued, notifying you that further action is required. In such cases, you will need to cancel the task assessment and return to it once the related action has been completed or resolved.
To progress the assessment, ensure that all the task prompts have "Yes" buttons selected, indicating that the required actions have been fulfilled.
Once all the "Yes" buttons have been selected for the task prompts, the "Next" button will become available for you to click. Click on the "Next" button to proceed to the next step or section of the assessment.
By following these steps, you can systematically assess the tasks, select the appropriate "Yes" or "No" buttons, and ensure that all the required actions are fulfilled before clicking on the "Next" button to progress with the assessment.
When systematically going through all the hazard prompts, please follow these steps:
Click on the appropriate "Yes" or "No" buttons to indicate your answer for each hazard prompt. Select "Yes" if the hazard is present or applicable, and select "No" if the hazard is not present.
If you click on the "Yes" button for a hazard prompt, you will be directed to the related hazard report. This is where you will provide additional details about the hazard.
Fill out the hazard report form with the required information. Provide a thorough description of the hazard, including any relevant details and supporting documentation.
Once you have completed filling out the hazard report form, click on the "Next" button to proceed to the next step or section of the assessment.
By following these steps, you can systematically assess the hazards, select the appropriate "Yes" or "No" buttons, and complete the hazard report forms for any identified hazards. Clicking the "Next" button will allow you to progress further in the assessment
Upon clicking the "Yes" button on the hazard prompt screen and being directed to the "Hazard Report" screen, please follow these instructions:
Provide more detail relating to the identified hazard in the relevant fields on the "Hazard Report" screen.
The "NAME" field will auto-populate with the logged-in username. If necessary, you have the option to change this field manually.
The "HAZARD TYPE" field is auto populated based on the hazard prompt type for which you clicked the "Yes" button. This field should already display the appropriate hazard type.
Fill out the remaining form fields, providing all relevant information and details related to the hazard. Ensure that you accurately describe the hazard and provide any necessary supporting information.
To add a "Risk Rating" to the hazard, locate the "+" icon within the "RISK RATING" section. Clicking on this icon will allow you to include a risk rating for the identified hazard.
By following these steps, you can provide additional detail regarding the hazard, make any necessary changes to the "NAME" field, fill out the form, and add a risk rating by clicking the "+" icon within the "RISK RATING" section.
To assign a risk rating to the related hazard report, follow these steps:
Read the "Potential Consequences" (top headers) and "Likelihood" (side headers) sections provided.
Based on your assessment, select the risk rating that best describes the severity of the hazard by clicking in the corresponding box where the consequence and likelihood intersect. This selection will determine the assigned risk rating.
After making your selection, you will be redirected back to the hazard report screen. Here, you will see your chosen risk rating displayed in the risk rating field, indicating your assessment.
Once you have reviewed and confirmed all the entered information, click the "Next" button to proceed to the "Actions" screen.
By following these steps, you can assign a risk rating to the hazard report by selecting the appropriate box based on the potential consequences and likelihood. Afterward, you can proceed to the next step by clicking the "Next" button, which will take you to the "Actions" screen.
On the Actions screen, you will find additional information related to the actions taken in the hazard identification process. Please follow these instructions:
To add pictures relevant to the hazard identification process, click on the camera icon. This action will allow you to capture or select images that provide further context or visual evidence.
Enter all the necessary information in the provided fields on the screen. Ensure that you include relevant details about the actions taken or planned to mitigate the identified hazards.
Once you have entered all the required information, the "Save" button will become available. Click on the "Save" button to save the entered information.
After saving, you will be directed back to the hazard prompt screen. On this screen, the "No/Yes" buttons will be replaced by a component displaying the count of completed hazard reports. This component serves as an overview of your progress in responding to the prompts.
Repeat the process for each hazard prompt, systematically responding to all prompts by clicking on the appropriate "No/Yes" buttons, entering information on the Actions screen, and saving the details.
Once you have responded to all the prompts, click on the "Next" button to proceed to the next step in the process.
By following these steps, you can add pictures, enter information about actions taken, save the details, track your progress through the hazard prompts, and click the "Next" button to move forward in the process.
Based on the different scenarios, here are the instructions for each situation on the hazard prompt screen:
If you had not selected any "Yes" buttons, meaning no hazards have been recorded, you will see a message stating, “No hazards have been recorded, continue to the next screen." In this case, simply click the "Next" button to proceed.
If you had selected "Yes" for any of the hazard prompts and set a risk rating of "Low" (green), you will see a message that says, "The risk rating is low for the above identified hazards, continue to the next screen." This message will be accompanied by details on the hazard type, risk rating, hazard description, and corrective action information. Click the "Next" button to proceed.
If you had selected "Yes" for any of the hazard prompts and set a risk rating of "Moderate" (yellow), you will see a message that says, "The risk rating is moderate or above for some of the identified hazards, STOP! See your supervisor. Further action required." This message will also include details on the hazard type, risk rating, hazard description, corrective action information, and any other hazards identified as "Yes." Although the message advises stopping and seeking supervisor intervention, you will still be able to click the "Next" button to proceed.
For high and extreme risks, you will be presented with the same screen as the "Moderate" risk scenario. Click the "Next" button to proceed.
By following these instructions, you can appropriately respond to the messages and prompts based on the risk rating and hazard identification and click the "Next" button to continue to the next screen.
On this screen, you will find a set of "No/Yes" options. Please follow these instructions:
Click on the appropriate buttons to select your answer for each set of options. Choose "Yes" if the criteria or conditions mentioned in the options are met, and select "No" if they are not.
If you select "No" for any of the options, it will prevent you from submitting your assessment. Make sure to review and provide accurate responses to all the options before proceeding.
If both sets of options are set to "Yes", indicating that the conditions are met, click on the "Submit" button to submit your assessment.
After submitting, you will be directed to the "Success" screen to confirm the successful submission of your assessment.
Finally, you will be redirected back to the home screen, where the dashboard will be updated with the new task assessment, reflecting the changes you have made.
By following these steps, you can select the appropriate "No/Yes" options, ensure that both sets are set to "Yes" for submission, and proceed to the success screen before returning to the updated home screen with the updated task assessment displayed on the dashboard.
