Risk Assessment User Guide

Loading Screen

Upon launching the application, you will be presented with a screen that pre-configures various aspects of the application, such as the initial data load. After a few seconds you will be redirected to the home screen.

Home Screen

The dashboard displays the count of job assessments conducted during the current week, month, and year as well as a count of open actions. This also includes a count of closed actions for the day.

Further to this, you are provided with a list of all open actions by risk assessment.

Risk Assessment Screen

The screen presents an overview of all risk assessments. To view a risk assessment, double-click on the row.

To add a new risk assessment, click on the "+" icon.

New Risk Assessment Screen - Scoping

To fill out the risk assessment form, follow these guidelines:

  1. Start filling out the form from left to right, following the provided sections and fields. Ensure you complete all required information.

  2. As you progress through the form, the submit button will become available once all necessary fields are completed.

  3. For the scoping section, you have the flexibility to select as many people as needed to form the team. However, keep in mind that only one facilitator can be selected.

  4. To minimize the scoping section, click on the "<" arrow. This will collapse the section, allowing you to focus on other parts of the form. To expand or maximize the scoping section again, click on the ">" arrow.

  5. Once the scoping is complete, a green tick or checkmark will be visible, indicating that the scoping has been successfully finished.

  6. To add events to the risk assessment, click on the "+" icon. This will allow you to include relevant events or incidents in the assessment.

By following these instructions, you can efficiently fill out the risk assessment form, ensuring all required fields are completed before submitting.

New Risk Assessment Screen - Events

To enhance clarity and ensure a smooth process in the risk assessment form, please consider the following instructions:

  1. The selected event will be distinguished by a darker grey color and a red border displayed above the "Triggers" section. This visual indication helps identify the currently selected event.

  2. To add triggers to the selected event, click on the "+" icon. This action enables you to link specific triggers that are relevant to the chosen event.

  3. If you wish to select a different event, simply click on the desired event from the available options. This will change the selected event, allowing you to associate triggers accordingly.

By following these instructions, you can easily identify the selected event, add triggers to it, and switch between different events as needed.

New Risk Assessment Screen – Consequences

Continuing with the risk assessment process, please note the following instructions:

  1. Once you have selected a trigger, you can proceed to add consequences and actions associated with that specific trigger. This allows you to outline the potential outcomes and corresponding actions in response to the identified trigger.

  2. Additionally, consequences can have controls added to them. To include controls, simply click on the control button associated with the respective consequence. This feature enables you to specify measures or safeguards that can mitigate or manage the identified consequences effectively.

  3. If, at any point, you wish to cancel the risk assessment and discard the progress made, you can click on the X icon. This action will cancel the assessment and any changes or inputs made up to that point will not be saved.

By following these instructions, you can effectively add consequences and actions to triggers, include controls for consequences, and cancel the risk assessment if needed.

New Risk Assessment Screen – Risk Rating

Assigning the risk rating in the risk assessment follows a simple process:

  1. Look for the appropriate risk rating by reviewing the labels or descriptions provided. These labels typically indicate the level of risk associated with a particular scenario.

  2. Once you have identified the appropriate risk rating, click on the corresponding rating. This action assigns the selected risk rating to the assessment.

  3. After assigning the risk rating, you will be returned to the new risk assessment screen, where you can continue with any remaining sections or finalize the assessment.

By following these steps, you can easily assign the appropriate risk rating to the assessment and seamlessly navigate back to the new risk assessment screen for further actions.

Risk Assessment Screen – Scoping

The risk assessment scoping is located at the top of the screen, allowing you to define the scope of the assessment and determine the team involved. Filtering in the risk assessment occurs from right to left, starting with event selection and then progressively narrowing down your focus.

Within the assessment, you have the flexibility to edit and add controls and actions by utilizing the pencil and "+" icons adjacent to them.

By following this user guide, you can effectively navigate and utilize the features and functionalities of the risk assessment application.
